I hereby give ESP permission to enroll me in Academic Prep: How to Be a University Student. I understand that the fees will be added to my student account at Carleton University and payment is due in full by August 25th, 2024. I accept all accompanying financial charges and am aware that non-payment or non-attendance does not eliminate the assessed fees and does not cancel my course selection. I understand that if I wish to withdraw and receive a full fee-adjustment I must email esp@carleton.ca by noon on August 14, 2024.

We may also follow up at a later date to obtain your feedback and opinions concerning the ESP, or to discuss enrolment opportunities into a post-secondary educational program offered by Carleton University.
